Medical Records

To obtain a copy of your medical records a release form must be signed. Your records will be sent to the address you specify. We do not charge for records sent to another physician for your continued care. However, charges may apply in other cases such as medical records requested by an attorney or by an insurance company evaluating your application for coverage. Please allow 10 days to process the request. If you have questions, please call (520) 290-0300 and ask for the Medical Records Department.
